REC is the scariest horror movie I've ever seen, and I've seen pretty much everything on the list (Why in god's name is REC not on the list?). Not Quarantine, it was fairly mediocre, the original Spanish one with the really hot reporter girl.
Like REC, Martyrs seems to fit into your list quite nicely. It is part of French New Extremism, a movement you would no doubt be interested in. Gaspar Noé, the director of Irreversible, is also associated with this movement.
As far as movies like Gummo - Other Harmony Korine films have a similar brutal intensity, particularly Julian Donkey Boy and Kids. Poverty movies like Winter's Bone have a desolate feel, and the world portrayed is pretty grim. Films like Antichrist and Breaking the Waves, and other Dogme 95/Lars Von Trier movies have a certain quality about them that you may enjoy. Antichrist is particularly brutal.
If by horrifying you mean emotionally draining (as I would imagine from some of your films on the list) I would consider watching Alan Clarke's films Scum and Elephant. Neither are horror movies, but both are brutally violent and very well made, and I believe have lapsed into the public domain. In any case they are available on Youtube (some of the scenes from Scum were missing though, or heavily edited, so you may want to find a different source). Gus Van Sant's film Elephant (about school shootings) took its steady-cam approach from these two films.
Another good one that is maybe a bit lesser known is Repulsion, a Polanski movie from the 60's. It is a subjectively filmed movie about a woman who descends into madness - you see things from her perspective - so the world slowly warps around her. It contains in my opinion the best jump moment of all time, which I won't spoil, but there are several and the majority are very effective and clever.
As far as brutal war movies (you listed Holocaust movies in OP) nothing comes even remotely close to Come and See (Иди и смотри). It's a Russian movie about the destruction of a small village by the Nazis. I must emphasise that nothing comes even remotely close to this, out of every other war movie ever made. Absolutely nothing. In terms of quality and content.
Also, Odishon and Blue Velvet are great films.
This is by no means exhaustive, but I would strongly recommend you look into some of these films. If you can only watch a few, I would recommend some of the lesser known ones: Come and See, Repulsion and Scum. They have a sense of brutality and tension that appear to be what you're looking for. I must echo this posters sentiment in regards to the work of Lars Von Trier and Harmony Korine. Movies like Julien Donkey-Boy are not "scary" in the traditional hollywood sense, but they convey intense, visceral conceptions of oddity/weirdness that cut deep. And even though its as cliche as it gets, I recommend Begotten to anyone looking for a piece of unsettling cinema. The odd sounds, unique camera technique, and veritably insane content are great for anyone looking for a movie to unsettle them.

'Teeth' is a movie that's pretty hard to watch. It's not super dark like most of the movies mentioned in the thread but it's still something that I think people who like disgusting/disturbing things would like.

I always loved in the mouth of madness. For a more recent movie, The Woman in Black was actually very unnerving, although not from the material (which is pretty tired), but the camera work.

I'm guessing the OP is looking more for disturbing/disgusting films rather than outright scary ones? I second REC for a scary film, and there are plenty of Japanese ones with freaky/disgusting scenes (Audition, Ichi, the Grudge). It sounds like the Human Centipede is the kind of film you're looking for, along with Cannibal Holocaust and I Spit on your Grave. And wasn't one of the Marquis de Sade's books made into a film in Italy?

Both the trailer and the movie are not all that great, but Salo is still worth watching imo. I am waiting for a contemporary to really do de Sade justice but we shall see :D

I think the most scared I've been of a movie was when I watched Event Horizon by myself at 2 in the morning with the lights off. I was around 11 at the time. Still haven't seen it again. lol
