Instead the cake was just completely pointless, Not only was it pointless, it was pretty underwhelming to see her eat it IMO. Like it was supposed to be ten times better than anything she ever tasted if we take for granted that sanjis the best cook, but her reaction wasn't really spectacular..

. Also, we have seen age-related illnesses in One Piece like with Lao G - who is so old that he can't see, he can't hear, and his legs are nearly giving out. There's also Whitebeard, a Yonko, who essentially had to be hooked up to an oxygen tank and plasma bag. + Show Spoiler + . She's only 4 years younger than Whitebeard was when he died. So honestly, that's how Big Mom should kick the bucket - the cake's sweetness kills her.

And I would understand if she had a quick metabolism like Luffy that all that sugar essentially gets used up too quick to cause problems, but her metabolism is certainly slowing down with age + Show Spoiler + . Hyperbola logic: Why doesn't this person have diabetes? Nevermind that eating sugar doesn't actually cause diabetes. Nevermind that the One Piece logic is a world where nobody ever had food related illness, can drink their own body weight in sake without stopping or dying. Can instantaneously fatten up from excess eating and losing said fat from some light excercise and some people can freaking fly, have wing, 2 elbows in one arm, are 20 meters tall, have 100 times the normal bodymass, snake necks and virtually every important character have magical powers. Clearly, the one thing that breaks your suspension of belief is that Big Mom doesn't seem to have diabetes. Even though it makes no logic in real life that she would have diabetes. Do you even understand what diabetes is, or are you just being deliberately obtuse? Shit, Hyperbola, what do you want? Oda to draw every panel with Big Mom stabbing herself in the finger?

What I'm saying is that it makes no sense for a 68 year old woman who is built like a cow and gorges herself on shit tons of sugar daily to not have any health problems when a 72 year old alcoholic is on his death bed. In terms of excess, sugar is far more damaging to the body than alcohol.

It doesn't make sense in real life, and it doesn't make sense in One Piece. It doesn't make sense that Big Mom exists, a ten meter tall person whose skin is tougher than steel, but what you are focused on is diabetes. You might as well ask, why doesn't Big Mom have bad teeth? In a world where people regrow their teeth the next panel. At least teeth rotting makes sense in real life. Also: "In terms of excess, sugar is far more damaging to the body than alcohol." lol That's a pretty common argument that people use when they don't understand world building - "Nothing is supposed to make sense because it's fiction." One thing that you have to realize is that world building has to have internal consistency to be any good. Once you establish the rules, you follow them. Once Piece established the rules that there is disease, and that old people have weaker bodies and can get sick from age-related illnesses. One Piece also established the rules that metabolism gets worse with age (as seen in many characters that get fatter with age). Hence, put two and two together, and you get Diabetes. Your example of teeth regrowth is an established rule in One Piece. Certain people can regrow teeth and therefore it's not unreasonable for someone who leads an unhealthy lifestyle like Big Mom to have good teeth. It doesn't rule out other health risks though. And yes, by the way, sugar (rather, glucose) and its aftereffects are basically the number 1 killer in 1st world countries. An excess of glucose leads to obesity, cancer, heart disease, and a whole bunch of other problems. Alcohol will destroy your liver and may make you fatter, but it kills you a lot slower than just plain overeating. We don't even know if that was the cause of Whitebeard's health problems either since he clearly wasn't fat. Maybe it was liver failure? Might of had some unrelated problems like failing lungs - hence the oxygen mask. Either way, it makes no sense for Whitebeard to have worse health than Big Mom - you simply can't argue against this.
